Summer Camp Forms & Information
MEDICAL INFORMATION for children NOT currently enrolled at A Child’s Place School
If your child is NOT a current school year student at A Child’s Place School, please complete this online MEDICAL FORM as soon as possible but before the first day of camp. The immunization record (and allergy plan if applicable) from your child’s physician may be submitted in one of the following ways:
1. Upload it into the area specified in the online medical form (above).
2. Email it to camp@achildsplaceschool.com.
3. Mail a copy or drop it off (ACP Summer Camp, 1409 W. Front St., Lincroft, NJ 07738)
If your child is a current school year student at ACP we will use the medical information and immunization record already on file.
